Diagnosis of Moderate to Severe Migraine
Headaches for at least a year
Residual Shingles Pain
18 years of age or older
Diagnosis of a shingles(herpes zoster) episode within a year and current persistent pain after the episode was gone
Schizophrenia
18yrs - 65 yrs of age
Diagnosis of schizophrenia for at least a year
